Salary Range: $52,000 to $55,000 plus opportunity to earn a quarterly bonus.

Community Care Liaison is a key position within our home care agency. Primarily work in the field assessing clients, mentoring caregivers and building referral relationships. This role with primarily be covering Maricopa County. Office is based in Chandler, AZ.

Responsibilities

  • Represent company in interactions with our care team members (caregivers), potential clients, current clients, clients family members and referral sources
  • Conduct in-home assessments, build care plan based on assessment and work with Scheduler on matching a caregiver to the client
  • Weekly follow ups with potential clients who are not ready to go on services in efforts to stay on their radar when care and support services are needed
  • Monitor the quality of care via follow up calls with clients, in-home supervisory visits, engaging with caregiver(s) assigned to the case and referrals to wrap around skilled health services for best outcomes
  • Care navigation - know all local resources and how to connect our clients to services and resources that are out of our scope of practice
  • Build referral sources and network to achieve assigned growth goals
  • Main support system for clients, caregivers and referral sources
  • Work in a team environment with Co-Owners, Recruitment and Retention Director, Scheduler and Caregivers
  • Document all assessments, follow ups and referral building activities in our operations system
  • On-call responsibilities

Qualifications

  • Must have 3 or more years of proven experience in social work, case management, home health, hospice AND/OR home care
  • Compassionate, kind, caring and be a great listener
  • Outstanding problem solving skills and ability to work in a fast paced business
  • Understanding of working in a continuum of care industry
  • Excellent follow through skills and self starter
  • Responsible for maintaining current referral relations and building new referral accounts
  • Representing our company at public events and professional network gatherings
  • Able to pass series of background checks, Level One Fingerprint Clearance Card, Proof of Negative TB, Current First Aide and CPR Card
  • Daily email to team on staffing needs and updates on new start of cares
  • Mostly in-field work with one in office day per week for team meeting and review of growth goals (office located in Chandler)
  • On-call responsibilities shared with Scheduler
