Job Description

We are seeking a highly skilled and compassionate Member Care Coordinator to join our dynamic team. As a Member Care Coordinator, you will play a vital role in ensuring our members receive the highest level of care and support, empowering them to take control of their health and wellness.

Overview

As a Member Care Coordinator, you will be responsible for conducting home health assessments, providing education and support to members, and ensuring they adhere to their personalized care plans. You will also serve as a liaison between our members, clinicians, and other internal teams, providing exceptional customer service and communication skills. Required Skills

  • Bachelor's degree in Social Work, Psychology, or related field
  • 1-3 years of experience in managed care systems, care coordination, or a related field
  • Bachelor of Social Work or Psychology OR LVN, LPN with 1 year experience in managed care systems OR RN OR 3 years care coordination for a state managed or waiver program OR 3 years managed care systems experience.
  • Knowledge of medical terminology and healthcare policies
  • Excellent verbal and written communication skills
  • Ability to work effectively in a fast-paced environment
  • Strong customer service skills and ability to motivate members to engage in their care plans
  • Proficiency in Microsoft Office applications and ability to learn our systems
  • Current driver's license, transportation, and applicable insurance

Nice to Have Skills

  • Experience coordinating member medical-related needs and analyzing member needs
  • Certification in a related field (e.g., CMC, CCM)
  • Experience with case management software and electronic health records
  • Bilingual or multilingual skills

Preferred Education and Experience

  • Bachelor's degree in Social Work, Psychology, or related field
  • 2-5 years of experience in managed care systems, care coordination, or a related field

Other Requirements

  • Ability to travel as required (approximately 10% of the time)
  • Must be able to maintain confidentiality and handle sensitive information
