JPNDC's Family Childcare program (FCC) is a system of 80+ independent licensed early educators offering childcare in their homes to low-income children with public childcare subsidies. JPNDC supports educators in providing high-quality early education and care designed to prepare children for success in school and life. We also support families through an array of wraparound services and enrichment activities.

The Role:

The Case Manager will join the Family Child Care team, reporting directly to the Sr. Case Manager for Family Child Care. As a dedicated and skilled professional, the Case Manager will serve as the family liaison for our priority populations, including DCF, young parents under 24, and families experiencing homelessness. In collaboration with various agency partners, the Case Manager will support individualized family needs, facilitate tailored Family Plans, and conduct developmental evaluations. This role is ideal for someone passionate about making a positive impact in the community by supporting families and children, particularly those with high needs facing challenging circumstances.

Duties:

• Liaison for our priority population such as: DCF, Young Parents Under 24, and Families Experiencing Homelessness.
• Participate in regular meetings with DCF, Shelter Systems or other family service agencies.
• Supports contractual needs of the children and families including referrals to community resources and multi-agency reporting as required.
• Responsible for facilitating cross department referrals with JPNDC Family Mobility Services to develop Individualized Family Plans tailored to each family's needs and goals.
• Performs developmental evaluations such as: ASQ's/ASQ-SE's for newly enrolled and families as needed.
• Facilitates and supports parent workshops, education, self-sufficiency, and other related enrichment activities for priority population.
• Provides families with ongoing support in navigating referral processes such as: Early Intervention, Public School Systems, Public Assistance Benefits, and/or Hospitals.
• Responsible for providing one-on-one assistance and referrals to supervisor to help identify high-needs families.
• Conduct routine home visits to educator programs and prepare related reports.
• Other duties as assigned by supervisor.

Qualifications:

Required:
• Minimum 1 year of direct service experience providing case management and/or counseling services to children, adolescents, and families in DCF, Shelter.
• Bilingual written and verbal (English/Spanish/Haitian/Cape Verdean Creole).
• Experience in one or more of the following: working with families of underprivileged socio-economic backgrounds working with multicultural populations meeting diverse family needs, stabilizing challenging circumstances, & supporting children and families who have experienced trauma.
• Knowledge of services and resources available to families within the community.
• Strong knowledge of Early Childhood Education, Social Work, or related Human Service field required.
• Sensitivity to and experience with handling confidential information and interactions, interacting in a culturally appropriate and responsive manner with a wide range of individuals.
• First Aid and CPR certified (certification training available).
• Able to use own vehicle for work daily.

Preferred:
• Associates degree or equivalent Case Management, Social Work or related Human Services curriculum certification.

Employment Terms/Compensation:
This is a full-time, salaried position with a generous benefits package. Salary range is $65,000 - $75,000.
